The Community Health Navigator will provide culturally appropriate care coordination, health education, and outreach, acting as an advocate and liaison 1. The Community Health Navigator will act as an advocate and liaison for under resourced individuals and families in the BILH community. The Community Health Navigator will assist patients who are experiencing various social and medical gaps in care and will help patients to access services available to maintain and improve wellness.

The Community Health Navigator will work alongside clinical teams in specific assigned community practices within BILH to promote health and wellness and contribute to the goals and objectives of the patient’s care plan. To be successful in this role, the Community Health Navigator will be a people person and able to develop relationships with patients, physicians and other care team and community members in this developing program.

Job Description:

Essential Duties & Responsibilities including but not limited to:

  • Effectively work with people from diverse backgrounds to reduce cultural and socio-economic barriers to quality health and social services

  • Establish positive and supportive relationships within an assigned caseload and work to motivate patients and caregivers to be active, engaged participants in their health.

  • Provide peer support and advocacy for patients and their caregivers, helping them navigate the healthcare system and meet both personal and clinical goals based on shared decision making.

  • Working under the guidance of the clinical team, within the primary care milieu, facilitate communication, access, and services between providers, local resources, and patients and their caregivers.

  • Support patients and caregivers in addressing language and cultural barriers to care and self-care

  • Coach and guide patients to meet both personal and clinical goals based on shared decision making.

  • Conduct outreach calls to encourage patients and caregivers to engage in coordination of care activities. ‘Meet people where they are’ according to their needs, this may include accompaniment to provider appointments, in the patient home or other centrally located community based visits

  • Conduct social need, screenings and assessments to identify barriers and support  plans to reduce the identified barriers.

  • Learn and become knowledgeable about resources to serve as a resource expert to facilitate access to community and governmental service agencies, which may include submitting referrals, completing paperwork on behalf of the patient and accompany patient when necessary.

  • As needed, participate in meetings and trainings with care teams

  • Willingness to engage in the local communities as identified as operational areas of focus, which may include Boston, Chelsea, Dorchester, Quincy, Randolph, Milton.

  • Flexible scheduled working hours, primarily week day and some evenings, opportunity to entertain hybrid remote work after successful onboarding / orientation period and satisfactorily meeting key performance measures.

  • Participate in departmental and/or interdepartmental quality improvement activities.  

  • Participate in and successfully completes Mandatory Education. 

  • Perform all other duties as needed or directed to meet the needs of the department.


Minimum Qualifications
Education:

  • At a minimum High School Diploma or GED required

  • Vocational or Technical training in Community Health Worker desirable

Licensure, Certification & Registration:  

  • No licensure or certification is required.

  • Medical Assistant, CNA desirable

  • Opportunity to explore certification as a Community Health Worker after 1 year.


Experience:  

  • 1-2 years related work experience in a medical office, or other human service / community based setting

  • Ability to work both independently and collaboratively within a team

  • Ability to travel (car or public transportation) within assigned community setting as needed


Skills, Knowledge & Abilities:  

  • Multi-lingual fluency preferred (Spanish, Cape Verdean Creole, Haitian Creole, Vietnamese, Cantonese, Mandarin). 

  • Basic knowledge of medical terminology preferred.

  • Ability to connect with people and understand the challenges they face

  • Team Work: Ability to interact respectfully with other employees, professional staff and/or external contacts to offer ideas, identify issues, obtain information or deliver services.

  • Customer Service: Ability to provide a high level of customer service to patients in a professional, service-oriented, respectful manner using skills in active listening and problem solving. Ability to remain calm in stressful situations.
