Job Description
Analyzes inpatient and ambulatory surgery records for deficiencies according to Medical Staff Bylaws and JCAHO guidelines for completion of records. Records deficiencies on deficiency slips and in computer (Meditech function) on at least 25 records a day. (Standard: accuracy of no more than two mistakes on physician's name and no more than eight mistakes on the deficiencies identification per month).
Reviews records of discharges from the floors, ambulatory surgery, and some outpatients to ensure that all documents are present and in proper sequence. Checks that documents are in the correct record.
Responds to the physicians who want to complete their records. Prints incomplete record list and pulls records from the incomplete cabinet for physician completion. a. Files pulled records back to the incomplete records cabinet if physician failed to come down within 48 hours.
Reviews records put into the incomplete file for reassignment. Updates the deficiency function in the computer and reassigns the location (Meditech).
Checks records of and provides names of the physicians to be put on the delinquent letter every Thursday. Prints and distributes incomplete records letters weekly to all physicians with records that need to be completed.
Maintains log of physicians with delinquent records and log of physicians who are sick or on vacation. Enters the information into the Meditech computer.
Performs final record check for completeness prior to permanent filing. Deletes records from Meditech computer.
Receives, maintains, and verifies census, transfer, admission, and discharge information, making corrections by comparing with the medical record.
Files pathology reports, cardiac catheterization reports, and GI records in a timely manner.
Performs other related duties as required or requested.
Required Qualifications:
High school education or equivalent.
Two years' experience in a medical record department. Will be expected to attain proficiency using multiple computer functions and learn the required elements of the medical record according to Medical Staff Bylaws and outside regulatory requirements.
