Mount Auburn Hospital (MAH) is a 200-bed acute care teaching hospital affiliated with Harvard Medical School. The Inpatient Case Manager (CM) performs utilization reviews and coordinates discharge planning for MAH inpatient admissions. Inpatient Case Manager performance is evaluated using the Inpatient Case Manager Guidelines and audit tool. Reports to Inpatient Case Management Supervisor.

Job Description:

Essential Duties & Responsibilities (including but not limited to): 
1.    Using established clinical criteria, the Nurse Case Manager identifies and communicates to the team opportunities for providing care in the most appropriate setting.   
2.    The Case Manager works with physicians, staff, patients, and families to develop a post-hospital plan of care that best meets the patient’s needs. 
3.    The Case Manager provides appropriate clinical information to the health insurer to ensure appropriate admission status and payment.  
4.    The Case Manager is the liaison between post-acute providers and the patient.  
5.    The Case Manager is part of the department’s weekend, holiday, and beeper rotation schedule as needed
6.    The Case Manager adheres to all insurance, contractual, and regulatory requirements as required.
7.    The Case Manager abides by MAH Case Management Standards as outlined in the MAH guidelines
8.    Other duties as assigned. 

Minimum Qualifications: 
1.    Bachelor’s degree preferred.
2.    Current Massachusetts state license as a Registered Nurse (RN).
3.    Two years of recent acute hospital medical/surgical nursing care experience are required.
4.    Recent case management experience is desirable.
5.    Intermediate computer and keyboarding skills.
6.    Effective communication, problem-solving, and negotiation skills.
7.    Demonstrated ability to organize and work independently in a fast-paced environment.

Physical Requirements & Environment: 
1.    Sufficient clarity of speech and hearing, with or without reasonable accommodation, which permits the employee to communicate well with the supervisor, community, employees, and general public
2.    Sufficient vision, with or without reasonable accommodation, which permits the employee to produce and review a wide variety of materials, written correspondence, reports, and related materials in electronic and hard copy form.
3.    Ability to lift, move or carry objects up to 10 pounds regularly.
4.    Ability to ascend or descend stairs or ramps using feet and legs, hands and arms. 
5.    Ability to maintain body equilibrium to prevent falling when walking, standing, or crouching.
6.    Ability to travel to and from various locations.
7.    Ability to learn and carry out the primary responsibilities of the assigned position.
8.    Ability to effectively communicate in writing and orally on job progress and other assignments and responsibilities.
9.    Ability to conceive and articulate plans, create and manage detailed projects, and organize and prioritize work.
10.    Ability to analyze data and exercise sound judgment in making decisions and managing confidential or sensitive information.
