This is an on-site role. The Health Information Department is a service department supporting complete and accurate medical record documentation that supports quality patient care. Accurate assignment of ICD-10 and CPT coding supports a complete patient record, statistical reporting and hospital reimbursement.

Job Description:

Job Summary: Duties include chart assembly, discharge analysis, incomplete records and chart completion/location. Performs record retrieval/delivery, filing, chart creation and answering phone requests. Provides back-up for chart mergers and resolution of the transcription error report. Assists with requests for medical record information (ROI).

Essential Responsibilities:

Assembles all medical record types according to standard chart order.

Utilizes Meditech discharge lists to monitor completion of all daily discharges.

Reviews assembled record for required contents and accurately completes the medical record completion form for any missing items (includes reviewing Meditech PCI for dictated reports that may need e-signature).

Records accurately any physician incomplete record item into the Meditech Incomplete Record program.

Records and reprints any missing record documentation such as the Laboratory Discharge Summary, Radiology reports and Nursing Documentation.

Required Qualifications:

High School diploma or GED required. Associate's degree preferred.

0-1 years related work experience required in Knowledge of anatomy/physiology and medical terminology preferred. TD-4 filing and knowledge of Federal/State medical record completion requirements preferred..

Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.

Oral Communications: Ability to comprehend and converse in English to communicate effectively with medical center staff, patients, families and external customers.

Knowledge: Ability to demonstrate full working knowledge of standard concepts, practices, procedures and policies with the ability to use them in varied situations.

Team Work: Ability to work collaboratively in small teams to improve the operations of immediate work group by offering ideas, identifying issues, and respecting team members.

Customer Service: Ability to provide a high level of customer service to patients, visitors, staff and external customers in a professional, service-oriented, respectful manner using skills in active listening and problem solving. Ability to remain calm in stressful situations.

Age based Competencies:
Employees in this job must be competent to provide patient care to the following age groups: Neonatal:Birth to 6 months, Youth: 6 months to 16 years, Young adult: 16-30 years, Middle Age: 30 - 60 years, Elderly: 60 -.
Physical Nature of the Job:
Light work: Exerting up to 20 pounds of force frequently to move objects. Some elements of the job are sedentary, but the employee will be required to stand for periods of time or move through out the hospital campus
