Patient Navigator - 40 hrs/wk, days
Are you looking for a growth opportunity in healthcare but aren't sure where to start? Anna Jaques Hospital has a new role that might be a great fit for you if you are an organized "people" person with some experience in care coordination within a healthcare setting.
The goal of the Cancer Center Patient Navigator (PN) is to navigate newly diagnosed patients to receive timely, patient-centered care at the Cancer Center. The PN will engage patients, create a trusting relationship, navigate patients to Cancer Center appointments, and closely communicate with a patient’s entire care team based on clearly outlined goals and identified patient needs. The navigator’s interventions might include education, appointment reminders; help with insurance, transportation, navigating the Cancer Center, assisting in finding appropriate child-care, interpreting, connecting the patient to psychosocial and/or palliative care teams, and physically escorting the patient to appointments.
Qualifications:
• Bachelors degree in health sciences or related field preferred. High School Diploma or equivalent required.
• Two to three years of experience working in community or health settings preferred but training will be provided for the right candidate.
• Demonstrated commitment to impacting the care of high-risk patients.
• Knowledge of complex patient issues preferred.
• Experience working as a patient navigator/community health worker preferred.
• Knowledge of the Core Competencies for Patient Navigator/Community Health Workers
▪ Ability to speak another language a plus.
