Job Description:

Position Summary: The social worker will provide behavioral health services to patients at BIDP presenting with substance use as part of a multidisciplinary Addictions Consult Team, funded by BSAS grant. Services include crisis intervention, intake, assessment, individual and group counseling.  The social worker will also provide case consultations to BID-P staff, work closely with ED and primary care teams, and assist with making referrals and ensuring connections to community based behavioral health providers. 

Duties/Responsibilities:

           Build strength based, clinically professional and therapeutic relationships with patients

•           Provide time sensitive crisis intervention, intakes, assessments, and treatment plans with patients and families

•           Assist with coordinating and providing telehealth resources

•           Provide evidence-based, solution focused, harm reduction interventions to patients and families, including individual, family and group counseling

•           Provide consultation to clinical staff upon request, including outreach and/or follow up as indicated

•           Participate in supervision, team meetings, and the creation of integrated plans of care for patients

•           Work closely and in a well-coordinated manner with BID Plymouth colleagues and community based behavioral health providers to ensure patients and their families receive access to timely, high quality care and services, including follow up to ensure connection

•           Participate in other activities related to BSAS and/or additional grant requirements such as internal and external collaboration meetings, trainings, and performance reporting

•           Provide and maintain up to date documentation in the electronic health record for billing requirements, team communication and/or as required by health insurers

•           Work to expand and enhance one’s knowledge of integrated care, best case practice for improving health outcomes, patient satisfaction, decreasing health care costs (toward achievement of triple aim), and advancing and operationalizing this care model within BID Plymouth and our partnering providers

•           Share clinical expertise with practice staff and other health disciplines to ensure continuity and quality of care

Education/Experience Required:

           Masters in Social Work from a CSWE accredited program

•           Currently licensed in Massachusetts as LICSW, LCSW, LMHC or LADCI (without restriction)

•           Experience providing strength based, harm reduction interventions with individuals struggling with addictions, especially those with opioids

•           Experience with short term, solution focused, cognitive behavioral and group therapies

•           Experience working with interdisciplinary teams

•           Knowledge of and experience working with suboxone, subutex and/or vivitrol

•           Experience using an electronic health record

•           Must be eligible for credentialing and contracting with all insurers.

•           Effective verbal and written communication skills to respectfully engage and communicate with diverse populations, including staff, patients, and families

•           Analytic sills to evaluate patient status and healthcare procedures/techniques and to monitor quality of care

•           Willingness to learn and remain flexible in the changing healthcare environment

•           Ability to conceptualize, develop and implement new ideas or systems

•           Ability to exercise sound judgment, discretion, and strict adherence to HIPAA while performing duties

•           Identifies, analyzes, and resolves patient care problems to foster significant improvement in patient care

•           Participates in interdisciplinary continuous quality improvement activities on an ongoing basis

•           Demonstrates a commitment to improving the quality of care provided to patients

•           Work requires reading, writing and checking documents for accuracy, opening attachments and inputting/editing words or data into or from a computer system (Microsoft office, word, excel)

•           A reliable car and the ability to drive  between offices and various meeting locations and for patient home visits

Registration/Certification:

Currently licensed in Massachusetts as LICSW, LCSW, LMHC or LADCI (without restriction)
