The Program Coordinator, Ambulatory Clinical Documentation Improvement will be responsible for supporting clinical documentation improvement initiatives, coordinating trainings, assisting with analyses, and troubleshooting day to day issues related to acuity/risk adjustment.

This will involve high levels of collaboration, integration, and communication across the Performance Network.

Job Description:

Essential Duties & Responsibilities including but not limited to:

  • Support a variety of initiatives managed under the clinical documentation improvement/acuity/risk adjustment umbrella, including, but not limited to concurrent CDI programs for risk adjustment, optimizing tools at point of care related to clinical documentation improvement, or building content to increase transparency and awareness related to acuity.

  • Coordinate trainings for clinical documentation improvement programs which can consist of trainings on understanding acuity or on specific tools within EMR applications

  • Assist with analyses related to clinical documentation programs, including, but not limited to lessons learned, opportunity, or assessing current program(s)

  • Collaborate with risk units and provider organizations across the network to enable best practices and understand needs

  • Integrate operations of the program into other value based areas as applicable

  • Host initiative check-in meetings with key stakeholders across the respective program and address follow up items as applicable

  • Performs data quality and validation checks for initiatives within identified and impacted applications (e.g. Arcadia, Athena, Epic)

  • Develop action plans and project schedules on areas identified that would optimize, refine, or integrate technological solutions/functionality where applicable into the respective EMR/Population Health applications across the network to drive performance both at the point of care and outside the visit. Examples of applications include Arcadia, Epic, Athena, and eCW.

  • Support and troubleshoot any issues related to recommended workflows from BILHPN related to acuity

  • Monitor outreach efforts within contracts relative to risk adjustment

  • Contribute to maintaining and developing a library of content and resources related to clinical documentation improvement/acuity

  • Compile colleague feedback related to risk data dissemination/reports and program(s) and carry out changes as applicable

  • Develop and maintain connections with vendors, where applicable, and other organizations to incorporate necessary discoveries into planning and strategies

  • Assists in the communication of initiatives related to acuity/risk to local leaders, administrators, support staff, and providers

  • Build and maintain working relationships with internal teams, practice operations, and provider community to foster collaboration and partnership in driving meaningful change within clinical documentation improvement

Minimum Qualifications:

Education:

A Bachelor’s Degree in Healthcare Management, Administration, Public Health, Business Administration, or other related functional area is required or a combination of equivalent education, experience, and certification.

Licensure, Certification & Registration:

CCDS or CPC preferred

Experience:

Minimum three years of program or project management, system implementation, clinical documentation improvement/coding, or clinical operations experience in a healthcare setting required.

  • Strong organizational skills

  • Experience with EHR clinical decision support tools and population health functionality preferred

  • Ability to work with data analysts and communicate needs

  • Excellent communication skills, both written and verbal

Skills, Knowledge & Abilities:

  Microsoft Word, Excel, PowerPoint, Project (preferred), EMR application knowledge, population health management software (preferred)
