MassHealth, Massachusetts Medicaid program, operates an Accountable Care Organization (ACO) program as part of a long-term, statewide effort to shift Medicaid reimbursement away from a fee-for-service model and toward accountable care.

BILHPN Masshealth-ACO is responsible for improving the overall quality and coordination of patient care, while reducing unnecessary hospitalizations and readmissions, emergency department visits and revisits. BILHPN Masshealth ACO will provide services in collaboration with contracted Managed Care Organizations (MCOs) as well as Community Partners (CPs) for Behavioral Health (BH) and Long-Term Services and Supports (LTSS), and Flexible Services Programs as appropriate.

The Social and Behavioral Services Program Manager will collaborate with leadership on the behavioral health and social services alignment for the MassHealth ACO. With the assistance of the ACO Program Manager, and input of the Executive Director and Medical Director, this role is responsible for administration of the Community Partner and Flexible Services programs, including design, adjustments, budget setting and tracking, related referrals, patient assignment, tracking, internal and state required reporting, and management of relationship with external vendors (behavioral, disability, and social service providers). Achieves performance improvement in Population Health and value-based contracts by preparing and executing action plans and collaborating with multiple stakeholders within BILHPN, risk units, hospitals, providers, and leadership teams.

The program manager reports to the ACO Executive Director, with a strong dotted line to the ACO Medical Director for all clinical components of his/her work.

Job Description:

  • Experience:

    Social services, CHW, or other similar clinical experience strongly preferred.  Manager/supervision experience.

  • Oversight of Community Health Worker (CHW) team to support the Masshealth ACO patients’ social services and behavioral coordination in the BILH system

Essential Duties and Responsibilities:

  • Responsible for all communication with Behavioral Health Community Partner programs and Long Term Services and Supports Community Partner programs, and Flexible Service vendors, with Program Manager support

  • Responsible for tracking, monitoring, and reporting on performance of CP and flexible services programs, with Program Manager support

  • Manage collaboration with primary care offices and providers in supporting patient social and behavioral health needs, with Medical Director oversight and CHW support

  • Oversight of patient screening, enrollment, interaction as needed to support CHW scope of work Develop and refine workflows of CHW’s to support ACO goals

  • Monitor Flexible Service program budget and suggest adjustments as needed to meet patient and program needs

  • Monitor CP & Flexible Service program dashboards (as provided by WellSense), and/or develop/supplement as needed, and report regularly to leadership on program performance

  • Participate in meetings with leadership within BILHPN, providers, MCO partner and the state as needed

  • Respond to state and MCO required deliverables Attend meetings and tracks and acts on takeaways and next steps

  • Support key meetings by collaborating with key presenters, preparing agendas, staffing the meeting, tracking attendance, and taking minutes; as requested

  • Schedule, prepare for, and attend project tracking and monitoring meetings as required; documents next steps and follows up with key project team members to ensure follow through

Minimum Qualifications:

Education:

Bachelor’s degree in business, healthcare, or related field required. Masters degree preferred.

● Strong proven leadership, project management, and execution skills.

● Superb communication skills, both written and verbal.

● Capable of leading the charge through creative thinking. Comfortable challenging the status quo while able to work diplomatically to keep stakeholders engaged.

● Understands context of assignments and tasks and able to be flexible and adaptable to changing roles & priorities.
