The Community Health Navigator is a trusted member of the community who helps patients’ better access and coordinate their health care. The goal of the Navigator is to assist patients with chronic disease get medical care and work on goals to remain healthy. The Navigator will provide support to patients and assist with real life issues that are interfering in their wellness.

You will work alongside the clinical team to promote health and wellness and contribute to the goals and objectives of the patient’s care plan. A successful CHN will be a people person and able to develop relationships with the care team, physicians and most importantly the patient.

**Opportunity to explore certification as a Community Health Worker after 1 year.
The Community Health Navigator will work alongside clinical teams in specific assigned community practices within BILH to promote health and wellness and contribute to the goals and objectives of the patient’s care plan. To be successful in this role, the Community Health Navigator will be a people person and able to develop relationships with patients, physicians and other care team and community members in this developing program.

Job Description:

  • Essential Duties & Responsibilities including but not limited to:

  • Effectively work with people from diverse backgrounds to reduce cultural and socio-economic barriers to quality health and social services

  • Establish positive and supportive relationships within an assigned caseload and work to motivate patients and caregivers to be active, engaged participants in their health.

  • Conduct social need, screenings and assessments to identify barriers and support  plans to reduce the identified barriers.

  • Learn and become knowledgeable about resources to serve as a resource expert to facilitate access to community and governmental service agencies, which may include submitting referrals, completing paperwork on behalf of the patient and accompany patient when necessary.

  • Willingness to engage in the local communities as identified as operational areas of focus, which may include Boston, Chelsea, Dorchester, Cambridge.

  • Flexible scheduled working hours, primarily week day , opportunity to entertain evening, weekend and hybrid remote schedule options after successful onboarding / orientation period and satisfactorily meeting key performance measures.

    Skills, Knowledge & Abilities:  

  • Bilingual fluency preferred (Spanish, Cape Verdean Creole, Haitian Creole ). 

  • Basic knowledge of medical terminology preferred.

  • Ability to connect with people and understand the challenges they face

    Experience:  

  • 1-2 years related work experience in a medical office, or other human service / community based setting

  • Related work experience with Community based organizations or public service agencies

  • Ability to travel (car or public transportation) within assigned community setting as needed

  • At a minimum High School Diploma or GED required (required)

  • Vocational or Technical training in Community Health Worker desirable(preferred)

    Minimum Qualifications
    Education:

  • 1-2 years related work experience in a medical office, or other human service / community based setting

  • Related work experience with Community based organizations or public service agencies

  • Ability to travel (car or public transportation) within assigned community setting as needed
