Advance Your Nursing Career In Our New Care Transitions Department
Attend our informational open house breakfast on Thursday, June 1st from 8:00 a.m. – 10:00 a.m. at BIDMC. Hear from our Senior Leaders including Jane Foley, Interim Chief Nursing Officer; Tracy Lee, Associate Chief Nurse for Care Transitions; and Lorraine Antelo, Nurse Director Care Transitions. Meet members of the current Case Management team and find out more about what their roles are like.
The Care Transitions Department is looking to recruit experienced nurses into this highly specialized area. The RN Case Manager partners with the interdisciplinary care team to facilitate the progression of care for the hospitalized patient. Together with the medical provider, the RN Case Manager collaborates with all members of the care team, focusing on the delivery of efficient, high-quality care. This position ensures the appropriate utilization of clinical resources with a goal of a safe and timely discharge for the patient.
We facilitate the transition into this new role through a comprehensive orientation program led by our Care Transitions Educator. You will learn from case management experts in both the classroom and clinical setting, and you will receive one-on-one coaching to prepare you to join this dynamic team.
To be considered for this role, candidates must have graduated from an accredited nursing program and have a valid MA nursing license.
