Recovery Support Navigator - Lawrence Community Behavioral Health Center (CBHC)

The Recovery Support Navigator (RSN) functions as a paraprofessional offering outpatient care management and adept navigation of social service systems for individuals diagnosed with substance use disorder and/or co-occurring mental health disorders. Primary objective of the RSN role is to engage clients within the treatment system, assess and address barriers hindering treatment continuation, and facilitate comprehensive access to all outpatient services essential for seamless reintegration into the community. Utilizing a solutions-focused methodology, the RSN program addresses barriers such as financial, legal, and medical issues to promote sustained recovery, ultimately aiming to decrease recidivism and relapse rates.

Job Description:

Responsibilities;

  • Manage individual caseload, delivering community-based case management services to aid clients in accessing treatment, housing, financial aid, and healthcare services.

  • Facilitate collaborative discussions with clients to explore treatment options, coordinate warm hand-offs to programs, and navigate insurance issues.

  • Educate clients on available treatment modalities, including outpatient programs and addiction pharmacotherapy options.

  • Coordinate care efforts with relevant contacts to navigate diverse systems and benefits.

  • Conduct intake assessments, safety plans, and develop goal-oriented intervention plans for clients.

  • Obtain and maintain service authorizations, assess service necessity, and request extensions as needed.

  • Provide temporary transportation assistance and connect clients with appropriate care providers.

  • Maintain regular communication with active clients and document session progress accurately and timely.

  • Conduct ongoing research for social service programs and participate in discharge planning.

  • When working with pregnant and/or parenting clients, collaborate on a Plan of Safe Care and address specific pre-natal/post-natal needs.

Education & Qualifications;

  • Bachelor’s Degree required. Psychology, Social Work, Criminal Justice, or related human services field is strongly preferred.
  • Must be able and willing to transport persons served in personal vehicle. As such, must have a valid driver's license, reliable vehicle, and ability to satisfy BILH BS approved driver policy. Travel and an ability to meet persons served in a variety of outreach settings is required.
  • Licensed and/or certified candidates preferred. (Ex: LADC, LSW, Recovery Coach).
  • Training, relevant work or lived experience in the areas of mental health and/or addiction/recovery preferred.
  • Bilingual applicants encouraged.

Schedule: FT - 40 hours, Mon-Fri (10:00am-6:30pm)
