Job Description:

Duties/Responsibilities

  • Assists client with activities of daily living and assists in instructed exercise program.
  • Maintains awareness of client’s physical, mental and spiritual well-being
  • Promotes client's self participation in ADL’s where appropriate
  • Enhances client’s muscular use by providing proper body alignment. Exercises extremities through active and passive ROM as indicated and ambulates with or without assistive devices.
  • Transfers clients per care plan.
  • Performs the following client services: bathing, assisting in skin care (reporting any skin changes), assisting in foot care, oral hygiene, shampooing, filing and cleaning nails, dressing, shaving, and monitoring variations of the client’s physical baseline.
  • Ensures adequate nutrition and hydration of client. Prepares meals, monitors intake, and assists in feeding when indicated.
  • Ensures clients general elimination is in normal pattern. Assists with use of commodes, bedpans and urinals. Monitors output when indicated; performs basic ostomy and incontinence care.
  • Reminds clients to take prescribed medications.
  • When instructed, takes temperature, pulse, respiration, and blood pressure.
  • Performs limited home management tasks in the immediate client environment, which may include: meal preparation, washing dishes, light laundry, light housekeeping, and bed making with linen changing.
  • Works collaboratively with other community health care providers.
  • Attends support groups
  • May provide lengthy service visits, extended hours or overnights.
  • Participates as a member of the Hospice Interdisciplinary Team to address problems.
  • Provides emotional support to assist dying patients.
  • Responsible for providing care in a skilled nurse facility.
  • Perform selected activities as written on the care plan under the supervision of the nurse or therapist including:
  • Ostomy care.
  • Dressing reinforcement.
  • Simple dressing procedure.
  • Apply splints, braces, ace bandages and elastic stockings.
  • Measures and records fluid intake and output

Responsibilities of the Hospice Aide:

  • Adheres to Agency infection control policies.
  • Participates in team conferences.
  • Completes the required twelve hour annual inservice requirements.
  • Maintains a professional relationship with and adheres to Agency confidentiality practice.
  • Reports changed behaviors or indications of client problems including: safety, abuse, neglect, and environmental hazards.
  • Maintains and submits accurate weekly documentation of time sheets/flow sheets.
  • Adheres to the client’s written care plan and informs the supervisor or case manager of any change in condition or family situation.
  • Seeks assistance and notifies the agency when a change of schedule is required or when there appears to be a safety risk for self or others.
  • Assists in personal care and home management relating to comfort, care, and safety of a client in the home.

HOSPICE AIDE’S MAY NOT:

  • Perform any duties or procedures that require the skill or education of a licensed clinician.
  • Make judgment or give advice on medical or nursing problems.     
  • Handle finances of clients or their representatives, including any banking functions, handling cash and valuables, or purchasing tobacco, alcohol or lottery tickets.
  • Administer/pour/dispense/instill any medications, including med patches, over-the-counter medications or MD prescribed medications.
  • Transport clients or family members
  • Perform housecleaning, except light housekeeping directly relating to client care.
  • Grocery shop.
  • Give haircuts or cut nails.
  • Apply sterile dressing.
  • Administer enemas, perform disimpaction, perform irrigation’s of any kind, including colostomy, urinary catheter, etc.
  • Perform gastric lavage or nasogastric tube feeding.
  • Apply heat in any form.
  • Care for tracheostomy.
  • Provide clients with their personal telephone numbers.
  • Work directly for clients on a private duty basis.

Perform any activity that will jeopardize a client, the agency or a staff member.

Education/Experience Required

  • Ability to read, write and speak English.
  • Dependability and a caring attitude.
  • Home Health or Hospice Experience preferred
  • Reliable transportation required

Registration/Certification

  • Home Health Aide Certificate required
