The Case Manager is responsible for assessing service needs; counseling, developing plans for access to services, liaison with client, families, programs, and service providers.
Job Description:
Essential Functions/Key Responsibilities:
1. Assumes individual case load which involves conducting individual intake evaluation and counseling sessions, providing referral to continuing care, and sharing responsibility for group programming.
2. Obtains and documents in a precise and timely matter all relevant intake material. Assesses patients’ continuing care needs, contacts appropriate aftercare resources and arranges for transfer or admission to appropriate rehabilitation facilities and/or liaison with self-help organizations.
3. Shares responsibility for education and other group programming, including recreation.
4. Responsible for all necessary paperwork and record keeping as directed.
5. Participates in client evaluations and case conferences.
6. When possible and/or requested, provided referrals for affected family members to self-help programs.
7. Coordinate client’s transportation to next level of care when appropriate.
8. Support with oversight in the storage of medication in the program when indicated and ensures compliance with regulatory expectations.
9. Identify and report medication events to the Program Director or designee.
10. Ensure medications are entered in EMR and disposed of properly and full chain of custody for all medications is recorded.
11. Supervises the health and safety of patients in the program by completing trauma-informed checks in the frequency set by the agency’s policies and procedures.
12. Promotes residents’ participation in program curriculum and recovery and recreational activities.
13. Maintains all required documentation including observation logs and resident belongings inventories.
14. Processes patient admissions, transitions and discharges upon request.
15. Other related duties as assigned.
Qualifications & Education:

A minimum of one year related experience and/or training in the area of addiction treatment.

Associates Degree in Human Services or related field preferred. HS diploma or equivalent required.

Valid driver's license in good standing preferred. Must be able to complete on-site CPR training/certification.

Must demonstrate ability to relate to a substance abuse treatment population in a sensitive manner, an interest and willingness to work in a multicultural setting and must share a customer service orientation.
Pay Range:
$21.77 – $26.54
The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
