Job Description:
Essential Responsibilities
Clinical Care
- Conduct comprehensive history and physical examinations.
- Develop, implement, and adjust individualized care plans.
- Order, interpret, and follow up on laboratory and diagnostic tests.
- Provide clear education and guidance to patients and families to support understanding and engagement in care.
Clinical Leadership & Education
- Serve as a clinical resource and educator for staff, offering support through formal and informal teaching, literature review, and participation in seminars or conferences.
- Share best practices and promote ongoing learning within the care team.
Care Coordination
- Coordinate patient care across disciplines to ensure safe, effective, and timely transitions and treatments.
- Act as a liaison among visiting nurse services, clinical teams, and consulting providers.
- Ensure care plans reflect the needs of the entire service and promote optimal continuity of care.
- Collaborate with physicians and surgeons for complex diagnoses or treatment decisions.
Documentation
- Document all clinical encounters clearly, accurately, and in a timely manner to ensure seamless communication and high‑quality care.
Team Collaboration
- Work in a strong interdisciplinary partnership with healthcare providers to identify and address patient and family needs.
- Model a team‑based, collaborative approach to care delivery.
Required Qualifications
- Master’s degree in nursing.
- Current Nurse Practitioner license.
- Basic Life Support (BLS) certification.
- 0–1 year of related clinical experience.
- Valid prescriptive authority through:
- Massachusetts Department of Public Health (DPH–DCP)
- Federal DEA
- Authorization to practice as an APRN (MA Board).
- American Heart Association – BLS Certification.
- Proficiency with electronic health records, web‑based applications, and Microsoft Office tools (Outlook, Word, Excel, PowerPoint, or Access).
Preferred Qualifications
- Experience in a teaching hospital environment.
- Specialty‑specific clinical expertise.
Pay Range:
$64.00 – $105.77
The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
