We are seeking per diem Case Managers for multiple locations including Danvers, Tewksbury and Lynn. Clients are provided with the medical and emotional support that they need during one of the most pivotal stages in the recovery process. Services include individual case management and group treatment services focused on assisting the client with accessing step-down levels of care. The Case Manager role is a great opportunity for an individual who wants to advocate for and serve individuals who struggle with addiction within a collaborative, established, patient-centered organization.
Job Description:
Responsibilities:

Obtains documents and all relevant intake materials. Conducts individual intake evaluation and counseling sessions,

Shares responsibility for education and other group programming activities.

Participates in client evaluations and case conferences.

Provides referrals for affected family members to self-help programs when applicable.

Assesses patients’ continuing care needs, contacts appropriate aftercare resources and arranges for transfer or admission to appropriate rehabilitation facilities and/or liaison with self-help organizations.

Coordinate client’s transportation to next level of care.
Qualifications:

Associate or bachelor's degree in human services or related field preferred

High School Diploma or Equivalent required.

A minimum of one-year related experience and/or training in addiction treatment.

Valid Driver’s License in good standing preferred.
Schedule: Per Diem willingness to work all shifts including days, evenings and nights on an as-needed basis. Requirement of 2 shifts per month.
Pay Range:
$21.77 – $26.54
The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
