Job Summary: The Clinical Coordinator of the Anticoagulation Clinic is responsible for ensuring the implementation of safe and effective medication therapy management and monitoring adherence to clinic policies and protocols. The clinical coordinator works with the clinic manager to lead and support all anticoagulation team members in the delivery of enhanced ambulatory anticoagulation services. As part of the anticoagulation clinical duties, the coordinator has the responsibility for managing patients enrolled in the Anticoagulation Clinic. The coordinator assists with clinical education activities, serves as preceptor to pharmacy students and residents, responds to drug information questions, engages in quality improvement projects, and reports out quality metrics on a monthly basis. The coordinator performs necessary services such as provider consultation, patient monitoring, patient counseling and medication management within the clinic.

Responsibilities
Essential Responsibilities: Works collaboratively with pharmacists, nurses, physicians, and leadership to develop programs that improve patient outcomes through appropriate medication management. In addition to direct patient care responsibilities, the Clinical Coordinator is also responsible for:
1. Developing and implementing anticoagulation policies and procedures.
2. Serving as a member of the local anticoagulation services committee to provide oversight to outpatient anticoagulation related policies and procedures.
3. Developing ongoing competency monitoring, and the development and evaluation of anticoagulation personnel.
4. Assisting with staff scheduling to ensure appropriate coverage for assigned areas and shifts.
5. Supporting employees' daily work activities.
6. Assessing appropriate drug information and literature resources, and provide information to patients, other health care professions, and peers.
7. Identifying and engaging in department quality improvement activities to improve patient care, medication use process, and pharmacy operations.
8. Participating as a preceptor for the LHMC PGY-1 and PGY-2 residency programs.
9. Training new staff in all areas of anticoagulation clinic.
10. Performing annual reviews for patients enrolled in the anticoagulation clinic.
Qualifications
Required
· Advanced degree (PharmD) or equivalent clinical practice experience
· License Massachusetts Pharmacist required or immediate eligibility
· 3-5 years related work experience required
· Must demonstrate effective verbal and written communication skills both within the department and with health care professionals outside the department
· Demonstrate computer literacy skills including web-based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access
· Additional qualifications may be required depending on the area the pharmacist is primarily assigned to work. Examples of additional qualifications could include basic life-support training or certifications
Preferred
· Residency training (PGY-1 and PGY-2 Residency Training) with emphasis on Cardiology/Anticoagulation/Ambulatory Care/Transitions of Care
· Previous experience in anticoagulation service or ambulatory pharmacy
· CACP or other pharmacy board certification

Pay Range:
$127,982.40 USD – $172,244.80 USD
The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
