In alignment with this mission, the Licensed Practical Nurse (LPN) functions as an integral member of the ambulatory care team. The LPN collaborates closely with nurses, medical assistants, providers, and other healthcare professionals to deliver efficient, high‑quality patient care.
This role contributes to and carries out the patient’s plan of care as developed by the interdisciplinary care team, ensuring safe, patient‑centered clinical practice. The LPN adheres to all departmental and hospital policies, standards, and professional values while supporting smooth daily clinic operations.
Training, evaluation, and competency oversight for this role are provided under the guidance of a designated Nurse Leader within the department.
Job Description:
Clinical Care
- Implement individualized patient care plans in alignment with clinician direction.
- Administer medications safely and accurately; document patient responses promptly.
- Immediately report any adverse reactions or concerning changes in patient status.
- Assist clinicians with exams, procedures, and treatments as needed to support high‑quality care.
Administrative & Patient Support
- Conduct follow‑up calls and help patients schedule routine, urgent, or specialty appointments.
- Support completion of preauthorization requests, prescription renewals, and other provider‑requested forms.
- Fax and route clinical information to appropriate recipients promptly and accurately.
Patient Education
- Participate in patient and family teaching as directed by clinical staff.
- Maintain up‑to‑date, accessible patient education materials in the clinical area.
- Provide patient education that is culturally sensitive, linguistically appropriate, and tailored to varying health‑literacy levels.
Clinic Flow & Coordination
- Help maintain efficient clinic flow to minimize delays and enhance patient experience.
- Partner with practice staff and medical assistants to ensure timely, coordinated care.
- Actively identify and prioritize urgent patient needs; alert clinicians when immediate intervention is required.
- Collaborate with the registration team to manage walk‑ins, schedule adjustments, and same‑day changes.
Team Collaboration
- Serve as an engaged, collaborative member of the healthcare team.
- Work closely with registered nurses, medical assistants, and practice staff to support all aspects of patient care.
- Participate in staff meetings, contributing to problem‑solving, workflow improvements, and quality initiatives.
Required Qualifications:
Vocational or Technical training in Nursing from a professional accredited Practical Nursing Program required.
License Licensed Practical Nurse required., and Certificate 1 Basic Life Support required.
1-3 years related work experience required.
Must have interpersonal skill to interact with patients and families in a multi-cultural setting.
Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.

Pay Range:
$33.00 – $46.21
The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
