Case Manager (RN)
Experienced Registered Nurse with a strong inpatient background in medical-surgical care, discharge planning, and care coordination. Skilled at collaborating with physicians, nursing, social work, patients, and families to support safe, timely transitions of care. Brings strong clinical judgment, communication skills, and the ability to manage complex cases in a fast-paced acute care environment.
Job Description:
Essential Duties & Responsibilities
Utilize established clinical criteria to identify and communicate opportunities for providing patient care in the most appropriate setting.
Collaborate with physicians, clinical staff, patients, and families to develop and implement post-hospital plans of care that best meet patient needs.
Provide accurate and timely clinical information to health insurers to ensure appropriate admission status determination and reimbursement.
Serve as the liaison between patients and post-acute care providers to support smooth transitions of care.
Participate in the department’s weekend, holiday, and on-call (beeper) rotation as required.
Adhere to all applicable insurance, contractual, accreditation, and regulatory requirements.
Comply with MAH Case Management Standards and guidelines.
Perform other related duties as assigned.
Minimum Qualifications
Bachelor’s degree preferred.
Current Massachusetts Registered Nurse (RN) license.
Minimum of two (2) years of recent acute care medical/surgical nursing experience.
Prior case management experience preferred.
Intermediate proficiency with computers and keyboarding.
Strong communication, problem-solving, and negotiation skills.
Demonstrated ability to work independently, manage priorities, and function effectively in a fast-paced environment.

Pay Range:
$79,268.80 USD – $204,318.40 USD
The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
