Your Role: You’ll lead home care services for a dedicated caseload—performing assessments, initiating rehabilitative procedures, and coordinating with physicians. You’ll be the bridge between the hospital and a successful recovery at home.
Requirements:

License: Current MA PT License.

Experience: 2+ years in PT; 1 year in Home Care/Public Health.

Skills: Proficiency in OASIS and BLS certification.
Pay Range:
$80,080.00 USD – $107,777.00 USD
The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
