Under clinical supervision, responsible for psychosocial screening, assessment, diagnosis, treatment, and follow-up for patients and families in assigned patient population. Collaborates with physicians, nurses, internal and external case managers, community agencies, and other health care professionals to implement safe and effective patient care. Reports to the social work manager / manager of clinical service area.

Anna Jaques Hospital is a not for profit, 83 bed community hospital serving the North Shore, Merrimack Valley, and southern New Hampshire. Our Adult Inpatient Psychiatric Unit is a 20 bed unit . We are supported by an engaged, compassionate interdisciplinary team focused on high quality outcomes, meaningful patient interaction, and a therapeutic environment with exceptionally low use of restraints. 

What You’ll Do

  • Promote a strong commitment to quality patient care and the mission of Anna Jacques Hospital

  • Conduct ongoing psychosocial needs assessments and advocate for patient needs

  • Develop and implement individualized and group treatment plans

  • Identify barriers to discharge or transitions in care while coordinating referrals and next steps

  • Maintain accurate and timely documentation

  • Facilitate clinical interventions to prevent relapse and ensure continuity of care

  • Educate patients and families on illness, recovery, and treatment options

  • Collaborate with treatment teams, outside providers, and reimbursement teams

  • Participate in clinical and administrative meetings that support exceptional patient care

What You Bring

  • Master’s Degree in Social Work

  • Current Massachusetts LICSW required

  • Demonstrated clinical leadership experience or potential

  • Strong knowledge of:

    • Case management and managed care

    • De-escalation and restraint management techniques

    • Motivational Interviewing and short-term treatment models

  • Excellent assessment skills and the ability to develop effective treatment plans

  • Organizational strength and comfort working in a fast-paced clinical setting

  • Ability to provide brief family interventions as needed

  • Experience in an inpatient psychiatric or behavioral health setting strongly preferred

  • Demonstrated knowlege of psychiatric conditions, treatment modalities, and crisis intervention

  • Strong clinical judgment, communication, and documentation skills

  • Ability to work effectively as part of an interdisciplinary care team

  • BLS certification or ability to obtain within the required timeframe

  • Ability to meet the physical and environmental demands of an inpatient psychiatric unit

Pay Range:

$40.00 – $51.74

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.  Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
