Reporting to Manager/Director, works under the direct supervision of a Registered Nurse to assist clients with personal care and activities of daily living following a clinically oriented care plan in compliance with agency policy. Works with patients in the home to promote and maintain their independence in their own home environment.

Job Description:

Key Duties:

a. Performs basic nursing procedures as indicated in competency assessment to include but not limited to taking and recording of vital signs, finger stick PT INR, simple, non sterile dressing changes, bed bath, sponge tub and shower bath, shampooing, nail and skin care, oral hygiene, toileting and elimination, nutrition and fluid intake, application of ointments, creams and topicals, reporting on skin conditions within State of New Hampshire LNA Scope of Practice.

b. LNA provides services that are ordered by the physician included in the plan of care, permitted to be performed under state law and consistent with home health training. Performs tasks and provides personal care to patients following the plan of care in a safe and effective manner complying with infection prevention and control policies.  Monitors condition of patient and reports changes to appropriate  personnel.

c. Performs transfers, range of motion, positioning, exercises, assistance with activities of daily living (ADLs), ambulation, functional activities and other basic nursing  care as instructed.

d. Completed competencies in one of the following areas: (1) finger stick PT INR, phlebotomy/lab draws; or (2) advanced wound care as instructed by RN and allowed by LNA scope of practice.

e. Performs accurate and timely documentation of patient care information on appropriate patient medical record forms and/or electronically.

f. As part of the healthcare team, facilitates professional and therapeutic relationships with all patients, families, and health care professionals. Maintain open communication with patient, patient representative, caregiver, and family and honors patient rights.

g. Successfully meets established regulatory, agency, and departmental competencies.

h. Determines proper functioning of medical equipment to assure patient safety, if appropriate.

i. Enhances professional growth and development through participation in educational programs, current literature, inservices, meetings and workshops.

j. Meets department obligations for scheduling, weekend and holiday coverage as needed. Understands and meets expected productivity, attendance and meetings and obligations for scheduling.

Education Requirements:

High School Diploma or equivalent

RHIT or equivalent preferred. Experience in medical records management required. Strong team building and collaboration skills necessary.

Pay Range:

$19.50 – $26.24

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.  Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
