Job Description:
The Opportunity
Join our busy, well-established Heart Failure Clinic. As a key member of our team, you’ll manage complex heart failure patients, design comprehensive treatment plans, and drive optimal clinical outcomes.
We are looking for a compassionate, hardworking NP or PA with a strong cardiology foundation and exceptional knowledge of advanced heart failure care.
Key Responsibilities

Direct Patient Care: Perform comprehensive assessments and diagnostic evaluations in an outpatient clinic setting.

Care Planning: Develop, implement, and adjust individualized pharmacological and lifestyle treatment plans.

Diagnostics: Order and interpret echos, EKGs, and lab work.

Collaboration: Work alongside cardiologists, nurses, and multidisciplinary teams.

Patient Education: Guide patients and families on HF management and medication adherence.

Program Growth: Support inpatient HF consults (as needed) and contribute to exciting program initiatives (e.g., palliative care collaboration, specialized amyloid/sarcoid clinics).
Qualifications

Licensure: Active Nurse Practitioner (NP) or Physician Assistant (PA) certification.

Experience:

Minimum 2+ years of outpatient cardiology experience (Required).

Prior experience in Heart Failure management (Strongly Preferred).

Skills: Strong clinical knowledge of complex cardiac conditions and exceptional communication skills.

Pay Range:
$133,515.20 USD – $181,563.20 USD
The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
