The Hospice Triage Nurse
Provides appropriate palliative and hospice care through telephonic support, assessment, teaching, counseling to patient and/or family members. Coordinates care with RN Case manager and IDT

Job Description:

  Essential Duties & Responsibilities including but not limited to:

•    Provides telephone triage for symptom management and follows up to determine effectiveness
▪    Provide documentation of services utilizing problem-oriented record.
▪    Plan for continuity of care with nursing team members, patient, family, and other disciplines.
▪    Use opportunities for teaching nursing care, comfort measures and health practices to the patient, families and others.
▪    Communicate and coordinate with other health and social agency personnel to assist in planning, implementing, evaluation service to the patient and family.
▪    Coordinates Prescriptions that are requested, transcribes into electronic prescription software for MD signature
▪    Coordinates with contracted hospice pharmacy regarding delivery, enters medication profile into prescription software, faxes face sheet, and medication list to pharmacy

Patient/Family:

Assists RN Case manager with patient/family including but not limited to: 
▪    Counsels the patient and family in meeting patient needs related to physical, emotional, and spiritual needs. 
▪    Respects and supports patient and family’s cultural beliefs and values, individual needs, health goals and treatment preferences.
▪    Provides information to the patient and family regarding the dying process and expectations. 
▪    Provides instruction regarding medication actions, side effects, dosage, route, and frequency.
▪    Assists with obtaining prescriptions and refills as needed 

Communication
▪    Communicates with the physician and the Hospice RN Case Manager regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.
▪    Communicates and coordinates with the RN Case Manager visit needs of patient as appropriate. 
▪    Collaborates, communicates, and cooperates as appropriate with other health care providers to ensure quality end-of-life care.

Documentation
▪    Documentation supports hospice eligibility.
▪    Documentation includes all recommendations and follow up 
▪    Completes documentation in accordance with agency timeliness policies.

Professional Development 
▪    Maintains and updates clinical knowledge and skills based on current nursing practice.
▪    Adheres to departmental requirements for required training.
▪    Identifies specific learning needs & goals and collaborates with nursing leadership in developing a plan to meet them.

Additional Duties
▪    Participates in the weekend/holiday/on-call rotation as defined by BILH at Home. 
▪    Ensures that arrangements for equipment and other necessary items and services are available.
▪    Participates in in-service programs and teaches and precepts other nursing personnel as assigned by Clinical Manager.

•    Perform all other duties as needed or directed to meet the needs of the department.

  Minimum Qualifications: 
Education:    •    Graduate from a Licensed Practical Nursing program accredited by the National League of Nursing (NLNAC).

Licensure, Certification & Registration:    •    Current licensure to practice as an LPN in the state of Massachusetts and New Hampshire
•    Maintains CPR certification.

Experience:    •    Minimum of 3-5 years nursing experience in hospice
•    Phone Triage Experience a plus

Skills, Knowledge & Abilities:    •    Demonstrates excellent observation and problem solving/critical thinking skills.
•    Well-developed verbal and written communication skills 
•    Essential computer skills to work with EMR.
•    Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
•    Self-directed and able to work with minimal supervision
•    Shows strong ability to organize and prioritize workload independently; nursing skills per competency checklist.

Pay Range:

$32.00 – $43.07

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.  Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
