Job Description:

Beverly Hospital (BH),and Addison Gilbert Hospital are located in Beverly Massachusetts and are part of the Beth Israel Lahey Health System. Beverly Hospital (BH) and Addison Gilbert Hospital (AGH) are community based hospital systems located respectively in Beverly  and Gloucester, Massachusetts.  They are diverse community hospitals offering a range of services inclduing adult and pediatric acute care, maternal and child health services, behavioral health inpatient and outpatient services, and multiple other outpatient programs and services.

The ACNO Quality and Care Coordination (ACNO) provides administrative direction and leadership for: Transitions of  Care/Case Management, Utilization Review / Denials management,  Social Work Services, Clinical Documentation Integrity (CDI) and Interpreter Services.   Responsibilities include, planning, developing, directing, organizing and coordination of all departmental, initiatives, improvements and care across the continuum.  In addition, the department director is responsible for aligning all financial and clinical department goals with the goals of the organization. The ACNO  works under the direction and leadership of the Chief Operating Officer with a dotted line reporting to the Chief Nursing Officer / VP Patient Services, functions as part of the ACNO team, and has a close working relationship with the Chief Medical Officer.

The ACNO provides leadership, works collaboratively and cooperatively and influences multiple hospital departments which includes but is not limited to:  Senior Leadership, Nursing Leaders and Departments, Performance Improvement, Physician leaders, Hospital Medicine physicians, Access Services, Health Information Management, Revenue Integrity, Patient Financial Services, Compliance, and the Performance Improvement team. 

This position supports performance improvement initiatives, value based performance goals and specifically patient experience in the domains of  Discharge Information and Transitions of Care, with an expectation target goals will be met or exceeded as set by the Beth Israel Lahey Health System and at the local level.  The ACNO focuses efforts on continually anticipating, discovering, meeting & exceeding internal & external customer needs, readily adjusts priorities to respond to pressing & changing customer demands. 

Patient flow and throughput through the hospital stay is dependent upon effective and efficient Transition of Care and Discharge Planning.  Establishing and executing Length of Stay (LOS) reduction, readmission management, prevention and reduction strategies are key components of this position. The ACNO

leads development and implementation of strategies related to transitions of care connecting the pre and post-acute care from the short term acute care setting.

The ACNOdemonstrates leadership competencies as outlined in the annual Director’s performance appraisal tool such as but not limited to:  action oriented; demonstrates business acumen, customer focus, entrepreneurial spirit, innovative and an effective listener and communicator.  Displays integrity and trust, able to motivate others, hires the best people from within or outside the organization and is data driven.  Maintains technical knowledge and skill within span of control as indicated.

Education:   Master of Science in Nursing or related health care field required

Licensure, Certification, Registration: Current license to practice nursing in Massachusetts required.  Certification in Case Management preferred.

Skills, Knowledge & AbilitiesMust have excellent interpersonal skills; self -motivated and self -directed; a change agent; ability to effectively influence and persuade others; Personal computer use with knowledge and use of Excel.

Experience: 

-Five years or more progressively responsible leadership experience within an acute care hospital environment including Care Coordination and Utilization Management.

- Demonstrated strength in management development, team building, program development and project management.

-Demonstrated ability to manage results in quality, patient satisfaction, employee relationships and financial performance/revenue cycle.

-Excellent physician relations skills

-Extensive knowledge of Medicare Conditions of Participation, Payor Rules and Regulations and the Revenue Cycle.
