Job Description:

Essential Role Responsibilities:

Case Management Support Functions

Demonstrates knowledge and understanding of the electronic SNF referral process.

Demonstrates ability to retrieve and print from Epic all clinical information needed for the SNF referral packet.

Demonstrates ability to evaluate a CM SNF referral and request additional information if indicated utilizing navihealth, our electronic referral program.

Demonstrates collaboration and cooperation with SNF’s and acute rehab facilities during the referral/screening process.

Demonstrates collaboration and cooperation with the Case Managers and Social Workers during the SNF/acute rehab referral screening process.

Executes the SNF/acute rehab packet without delay once referral is received.

Assists Case Management staff on the nursing unit with the assembling of the discharge envelope.

Demonstrates skills and ability to plan for and facilitate procurement of necessary durable medical equipment, supplies and resources to promote successful patient discharge outcomes.

Communicates with commercial payors to check the status of authorizations for post-acute placement.

Information/Office Management

Demonstrates skill in data entry and production of documents.

Demonstrates knowledge and skill for information management and retrieval.

Epic, Navihealth, MS Word and Excel proficiency.

Demonstrates ability to provide telephone coverage for the department.

Assists with other general data entry, retrieval and clinical duties as duties as assigned.
