The Director of Resident Care is responsible for the overall leadership, planning, organization, and direction of clinical and resident care services at The Herrick House. This role ensures that all resident care complies with Massachusetts EOAI (Executive Office of Aging and Independence) regulations, physician orders, and internal policies. The Director of Resident Care fosters a high standard of compassionate, person-centered care, supports families through clear communication, and oversees day-to-day clinical operations—serving as the Executive Director’s delegate when needed.

Job Description:

Clinical Oversight & Resident Care

· Conduct comprehensive initial nursing assessments of all prospective residents prior to admission, evaluating physical, cognitive, emotional, and social needs to determine care requirements and risk factors.

· Review and update resident assessments 30-days post-admission to evaluate the effectiveness of the initial service plan. Conduct subsequent reassessments at least every six months—or more frequently if a resident’s condition changes—to ensure individualized care plans remain accurate and effective.

· Collaborate with residents, families, physicians, and interdisciplinary team members to develop, implement, and monitor personalized service plans based on each resident’s evolving needs and goals.

· Perform ongoing clinical and functional assessments to identify changes in condition, promptly adjusting service plans as needed.

· Oversee all aspects of medication management, ensuring safe administration and full compliance with EOAI regulations and physician directives.

· Coordinate ancillary healthcare services—including physical therapy, occupational therapy, and specialty care—and manage emergency interventions or hospitalizations to ensure continuity of care.

Staff Leadership & Development

· Recruit, hire, train, schedule, and supervise all licensed nurses and Resident Care Assistants (RCAs), with a focus on regulatory compliance and competency.

· Provide coaching and mentorship to staff; conduct annual performance evaluations and support professional development.

· Plan and implement EOAI-mandated in-service education, including SAMM (Skills Assessment and Medication Management) training for RCAs.

· Chair the Quality Assurance Committee to assess care outcomes, identify areas for improvement, and implement corrective action plans.

· Ensure adequate staffing coverage based on resident census and acuity.

Regulatory Compliance & Quality Assurance

· Maintain current knowledge of EOAI and other applicable assisted living regulations; ensure department-wide compliance at all times.

· Monitor daily care delivery; investigate incidents promptly and ensure timely reporting to EOAI and Safe Spot.

· Maintain accurate, complete, and audit-ready documentation of all resident care activities.

Budget & Administrative Duties

· Develop and manage the Resident Care budget, including oversight of payroll through Kronos.

· Prepare and submit monthly budget variance reports; collaborate with the Executive Director on financial planning and cost control.

· Serve as “Manager on Duty” on a rotating weekend schedule to oversee all aspects of community operations.

Community Engagement

· Conduct assessments of prospective residents within 24 hours of receiving clinical documentation from the PCP/Provider to evaluate admission suitability.

· Participate in tours, family meetings, and outreach events to promote The Herrick House and its care services.

· Provide 24/7 on-call clinical support to ensure continuity and responsiveness of resident care services.

Qualifications

· Graduate of an accredited nursing program.

· Current Massachusetts Registered Nurse (RN) license (required).

· Minimum of 2 years of RN experience; at least 1 year in assisted living, home health, or long-term care preferred.

· Minimum of 1 year of nursing management experience, including hiring, scheduling, supervision, and coaching.

· Valid Massachusetts (preferred) driver’s license.

Skills & Abilities

· Strong clinical assessment and critical thinking skills.

· Ability to manage multiple responsibilities, prioritize effectively, and lead in a fast-paced environment.

· Excellent oral and written communication skills in English.

· Demonstrated leadership and team-building ability.

· Proficient understanding of EOAI regulations and best practices in assisted living.
