Provides physician services within the spectrum of hospice and palliative care. Responsible for the medical aspects of hospice patients. The Medical Director guides, directs, and is a care consultant to the hospice and palliative care staff, the patient’s attending physician and other providers to assure standards, quality, and excellence of care is provided. Fosters and maintains external relationships between the hospice & palliative care programs and other professionals and organizations engaged in health care services. Mentors and leads the associate medical directors and Nurse Practitioners

Job Description:

Essential Duties & Responsibilities including but not limited to:

• Certify and re-certify the patient’s terminal illness.
• Performs both hospice patient visits as well as palliative care consultations in homes and facilities as needed.
• The physician, in conjunction with the patient’s attending physician, is responsible for the palliation and management of the terminal illness, conditions related to the terminal illness, and other serious illness.
• Provides accurate and evidenced based care related to assessing, treating, and diagnosing patients with advanced progressive illness while promoting a healing environment.
• Develops and implements appropriate advanced disease treatment plans that effectively manage the patient’s needs creating a culture with optimal utilization of resources.
• Participates in the assessment of system medical staff learning needs as they relate to palliative care.
• Plans, implements, and evaluates educational programs for medical and nursing staff based on identified learning objectives and educates hospital medical staff on the use of palliative care and hospice services.
• Coordinates educational activities with other institutional and community programs.
• Responds to changes in clinical practice by planning, designing, implementing, and evaluating guidelines, protocols, and standards in collaboration with other staff.
• Participates in the process of program development for the hospice and palliative care programs.
• Participates in committees established for the purpose of coordinating and enhancing patient care.
• Offers expertise in selecting data elements that are clinically relevant for the patient and recognized as valid for the hospice quality assessment and performance improvement program.
• Supervises advance practice palliative nurses and provides clinical support.
• Leads an interdisciplinary hospice team (IDT).
• Participates in the IDT task of assessing patient’s progress towards goals and their continued eligibility for hospice at least every 15 days.
• Shares night and weekend call duties with other hospice and palliative care physicians as needed.

• Participate in and successfully completes Mandatory Education.
• Perform all other duties as needed or directed to meet the needs of the department.

Education:

• Medical Doctorate degree (M.D.) from an approved school of medicine.

Licensure, Certification & Registration:

• Possession and maintenance of a valid Massachusetts and New Hampshire State Physician’s license to practice medicine in the both states
• Current DEA licensure.
• Certification by the American Academy of Hospice and Palliative Medicine preferred
• Current Driver’s License.

Experience:

• Minimum of 5 years experience working within a hospice and/or palliative care program as Medical Director
• Expertise in end-of-life care, including advanced care planning, pain and symptom management of the seriously ill patient, and psycho/social, emotional care issues
