This position is one of three inpatient infectious diseases/ antimicrobial stewardship pharmacist position and responsibilities include but not limited to:- Rounds with ID consult service and evaluates patient pharmacotherapy for appropriateness of drug selection, dosage, dosage form, route, and duration, considering individual patient characteristics and clinical status.
- Identifies and implements education strategies to improve antimicrobial use across the hospital, promote initiatives and mission with valued stakeholders
- Triages non-formulary requests for antimicrobial therapies and manage the antimicrobial drug formulary
- Keeps tracking of antimicrobial use for monitoring and benchmarking (e.g., impact of interventions, prescribing habits, antimicrobial resistance patterns)
- Participates in reporting antimicrobial use and resistance patterns
- Maintain organization-approved policies and procedures and participates in the system Pharmacy and Therapeutic Subcommittee
- Precepts clinical trainees (ID PGY-2 pharmacy resident, PGY-1 pharmacy residents, ID fellows, pharmacy students, medical residents)
- Supports existing goals and projects while seeking new opportunities to improve the antimicrobial stewardship program
- Participates in the System Pharmacy and Therapeutics Subcommittee
- Leads and participates in collaborative research projects
Job Description:
1. Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and DrugAdministration, and the Drug Enforcement Administration. Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures.
2. Reviews and verifies medication orders with consideration, including but not limited to the following patient specific parameters: diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, drug interactions. Clarifies orders deemed to be inappropriate, incomplete, or otherwise unclear with the prescriber in a timely manner.
3. Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care.
4. Provides drug information to physicians, nurses and other health care professionals.
5. Checks and verifies sterile and non-sterile medications.
6. Supervises the activities of students, technicians, and residents within the pharmacist area of practice.
7. Promotes a culture of patient safety by submittingmedication related events and near misses.
8. Completes daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives and relevant reports.
9. Works collaboratively with all pharmacists, pharmacy technicians, and pharmacy personnel. Upholds a culture of respect and teamwork across the Pharmacy Department.
10. Precepts pharmacy students, resident rotations, andresident-led quality improvement projects. May serve as Residency Program Coordinator or Residency Program Director
11. Regularly participates in department initiatives including but not limited to supporting formulary stewardship, presenting new medication requests at Pharmacy & Therapeutics Committee, creating and maintaining policies and procedures, conducting assessments of workflows, and/or developing departmental tools and resources.
12. Supports a specific core area of practice through interdisciplinary education, ongoing specialty training and as a resource for Pharmacy staff.
13. Updates clinical guidelines and presents hospital wide educational initiatives.
14. Serves on hospital interdisciplinary committees or workgroups to improve process and quality outcomes.
15. Supports IRB-approved research activities, medication use evaluations, or cost-savings initiatives.
16. Other duties as assigned
Required Qualifications:
1. PGY 1 + 1-3 years OR PYG2 OR 3-5 years of experience in clinical specialty area
