Nurse Practitioner - Job_Description_Under_Construction

Job Description:

Essential Duties & Responsibilities including but not limited to:

1) Develop a comprehensive health plan for patients and families, including social and emotional needs as well as medical management.

2) Perform routine health examinations through baseline histories review of systems and complete medical histories and Physical

examinations. Differentiate normal results from abnormal findings.

3) Identify and manage specified acute illness, i.e. URI, UTI, vaginitis, etc.

4) Identify and manage urgent care, i.e. superficial wounds and lacerations, burns, sprains, minor fractions, etc.

5) Monitor and manage stabilized patients with chronic diseases, i.e. hypertension, diabetes, thyroid, etc.

6) Respond to and assisting the treatment of patients requiring emergency care.

7) Write prescriptions and medical orders’ dispense sample medications and administer injections and immunizations under

established guidelines. Utilizes Electronic Medical Record (EMR) for prescription refills within established guidelines

8) Perform telephone triage and evaluation of patient illnesses, problems and concerns.

9) Consult with collaborating physician on cases of concern.

10) Make referrals to outside health care specialists and provide health care management for HMO and PPO members; initiate

written referrals to insurance companies as necessary.

11) Assist with completion and/or review of completed forms, i.e. office visit and lab encounter forms, insurance and

disability/worker compensation forms and provide necessary signatures. Provide direction to medical record staff regarding

completion of forms and APS release.

12) Teach and counsel patients in preventative health care, i.e. basic nutrition, specialized diets, obesity, smoking, exercise,

sexuality, etc.

13) Assist discharged hospital patients with home treatment preparation and provide education to family members responsible for

providing home patient care.

14) Perform minor surgical procedures and provide follow-up care, i.e. suturing/suture removal, dressing changes, etc. in accordance

with prior training.

15) Perform routine procedures, i.e. collection of specimens for culture and vaginal or microscopic examination including PAP

smears as well as blood, urine and stool specimens.

16) Perform a variety of diagnostic tests including but not limited to timed vital capacity, audiograms, pulmonary function tests,

tympanometry, visual acuity, electrocardiograms and skin tests.

17) Conduct Nursing Home visits and at home visits as defined by the practice guidelines.

18) Communicate relevant patient data via dictation and/or provide complete written documentation in the medical record including

summaries of examinations, office visits, progress notes, telephone conversations, procedures and tests. Maintains patient

confidentiality.

19) Adhere to patient care standards as defined by collaborating physician and Medical Director.

20) Attend regularly scheduled provider meetings and serve on committees in order to develop and assist with planning, developing,

implementing and monitoring new or revised clinical programs within the group practice.

21) Participate in ongoing educational programs for Nurse Practitioners; maintain professional development through organizational

and community resources, conferences, workshops, etc.; maintain BCLS certification.

22) Maintain collaborative working relationships with all levels of staff.

23) Provide on-call coverage on weekends and holidays, based upon rotation, treats acute care patients in the office and triages in

collaboration with on-call physician.

24) Incorporates Lahey Clinic Guiding Principles , Mission Statement and Goals into daily activities.

25) Complies with all Lahey Clinic Policies.

26) Complies with behavioral expectations of the department and Lahey Clinic.

27) Maintains courteous and effective interactions with colleagues and patients.

28) Demonstrates an understanding of the job description, performance expectations, and competency assessment.

29) Demonstrates a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer

Service standards.

30) Participates in departmental and/or interdepartmental quality improvement activities.

31) Participates in and successfully completes Mandatory Education.

32) Performs all other duties as needed or directed to meet the needs of the department.


Minimum Qualifications:

Education: Graduate of an accredited Nurse Practitioner program.

Licensure, Certification, Registration: Eligibility for or current Nurse Practitioner license in the Commonwealth of Massachusetts. Must maintain BLS certification through American Heart Association.
