Under the direction of the manager or supervising clinician, the Community Resource Specialist conducts resource assessments and collaborates with social work and/or medical staff to obtain community resources for Medical Center patients and families. Within developed area of expertise, provides orientation and ongoing training related to community resources to staff, students, medical professionals, patients and families.Job Description:
Essential Duties and Responsibilities including but not limited to:
- Determines,eitherindependently and/or inconjunction withclinicians, theresources thatwould optimallymeetpatients' andfamilies' needs.Uses knowledge of localand state resources todevelopa multifaceted service plan to meetpatients' complex needs. Makes independentdecisions about what services to recommend toa patient/family.(essential)
- Maintains acaseload with multiple patientcontacts perday,includingphonecalls,paperwork andface-to-face contacts. Functions as the primary health care contact fora psychosociallycomplex patients whenneeded. Provides timely and appropriate documentationin patients' medical records (paper and electronic). - Advocates on patients' behalf in written andverbal communication.Assistspatientsandfamilieswith such tasksas accessing entitlement programs.Providesappropriate medical information asrequested byextended carefacilities, Department of TransitionalAssistance, etc. (essential)
- Conductsstaffandcommunity basedtrainingonresources.Providesupdatestostaffofproceduralrequirements and changes required by state and local agencies. (essential)
- Documents andadvisessupervisor andclinical staffofobstacles toobtaining services that requiresocial action.Develops liaisons with community and governmental agencies tofacilitate negotiation ofcomplex systems. Visits agencies when indicated to broadenknowledge and relationships. (essential)
- Incorporates Lahey Health Mission Statement and Goals into daily activities.
- Complies with all Lahey Health Policies.
- Complies with behavioral expectations of the department and LaheyHealth.
- Maintains courteous and effective interactions with colleagues and patients.
- Demonstrates an understanding of the job description, performance expectations, and competency assessment.
- Demonstrates a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer Service standards.
- Participates in departmental and/or interdepartmental quality improvement activities.
- Participates in and successfully completes Mandatory Education.
- Performs all other duties as needed or directed to meet the needs of the department.

Team Work: Ability towork collaborativelyin small teamstoimprove theoperations ofimmediate work group by offering ideas, identifying issues, and respecting team members.
CustomerService: Ability toprovide ahigh level ofcustomer servicetopatients, visitors, staffand externalcustomersin a professional, service-oriented,respectful manner using skills inactive listening and problem solving. Ability to remain calm in stressful situations.
Age based Competencies:Employees in this jobmust becompetent toprovide patient care to thefollowing agegroups: Youngadult: 16-30 years, Middle Age: 30 - 60 years, Elderly: 60 - over.
Social/EnvironmentalRequirements:
Work requirescloseattention totaskfor work tobeaccurately completed. Intermittent breaksduring thework daydonot compromisethe work.

Physical Requirements:
Light work: Exerting upto20pounds offorce frequentlytomove objects.Some elements ofthejobaresedentary, but theemployee will be required tostand for periods of time or move throughout the hospital campus
Thisjobrequiresfrequent sitting,walking,standing, FineManipulation usingboth hands,Keyboarduse. There maybe occasional climbing, Pushing/Pulling using both hands.
Minimum Qualifications:
- HighSchooldiplomaorGEDrequired.Bachelor'sdegreeinSocialWorkorHumanServicespreferred.
- License LicensedSocialWorkerpreferred.
- 1-3years related workexperience required.
- 2yearsofrelatedworkexperiencewithout arelatedBachelor'sdegree
- Experience withcomputersystems required,includingwebbasedapplicationsandsomeMicrosoft Officeapplications which may include Outlook, Word, Excel, PowerPoint or Access.
