Job Description:

Reporting to the Director of Hospital at Home, the Clinical Operations Manager, Hospital at Home will oversee, optimize and grow the daily multidisciplinary operational infrastructure supporting the BILH Hospital at Home program.  This leader will partner with leaders across the health system throughout the growth phases of the Hospital at Home program to leverage learnings to improve care delivery to acutely ill patients in the home setting.  With exceptional patient safety as a guiding principle, the Operations Manager will also manage with a lens toward resource stewardship and efficiency.

The position is accountable for Operational, Financial, Clinical, Quality and Business development activities in partnership with senior leadership.  As regulatory requirements evolve, the position is responsible for creating and maintaining structure to ensure exceptional adherence to regulatory standards.

Duties and Responsibilities:

  • Primary focus of this role is to oversee daily Hospital at Home operations including optimal patient flow planning across the care continuum from the hospital to the care delivery in the patient’s home.  This includes coordinating the best care delivery plan daily balancing resource planning and each patient’s clinical needs.  This leader would support the operations 5 days/week while also creating the best processes possible for the same efficacy in the “off hours”.  It is known that over time as the program grows the “off hours” structure for flow coordination will likely need to change, and this leader will be charged with building and overseeing that structure.

  • In managing the daily operational work for care delivery to the patients, this leader will focus on working with the Hospital at Home nursing team primarily in partnership with others coordinating logistics for patients, physicians and APPs.

  • This leader will also support daily operations for the nursing team in concert with the flow coordination priority.  This includes rotational engagement directly overseeing operations in the “off hours”.

  • Build structure to support daily operations as the program grows over time.

  • Support iterative changes to the daily schedule or processes needed based on patients’ clinical needs.

  • Lead or designate leadership of multidisciplinary rounds (ensuring care coordination for patients including clinical and operational)

  • Optimize field schedule to support admissions / intake as well as care for the patients on service

  • Remove barriers to timely care delivery

  • Achieve these goals while optimizing resource stewardship

  • Create and update policies and procedures in partnership with the multidisciplinary team.

  • Partner with the rest of the Hospital at Home leadership team to manage relationships with vendor partners.

  • Manage RCA process as needed. 

  • Monitor all financials with the rest of the leadership team including but not limited to; budget, accounts receivable and financial projections. Ensure that the program meets and/or exceeds volume growth initiatives and all performance metrics. 

  • Build, sustain and grow systems within the Hospital at Home program to address social determinants of health.

  • Ensure optimal capability to serve patients of all ethnic and cultural backgrounds.

  • Serve as direct operational partner to operational and clinical leaders of each acute care hospital served by the Hospital at Home program. 

  • Facilitate and monitor quality and safety oversight of Hospital at Home services.

  • Partner with other leaders to manage communications strategy across BILH.

  • Display consistent behavior that enhances the public image of the organization as evidenced by professionalism and excellence in interactions with peers, direct reports, and those the program serves (patients, families, clinicians, payors, vendors, etc.)

  • Travel-Periodic travel for the purpose of outreach, marketing, and program growth.

  • Maintain strictest confidentiality in areas of patient, employee and clinician relations.

  • Employee maintains competencies (position specific and organization-wide) required for the specific patient population served.

  • Perform all duties in accordance with safety and other laws, rules and regulations as set forth by appropriate regulatory and government agencies and in accordance with established department and hospital policies and procedures.

  • May be required to perform similar or related duties reflective of the level of experience as may be necessary on an emergency, temporary or limited basis or as designated by the of Hospital at Home leadership team or their designee.

  • The nature of the tasks/responsibilities, required of employees in this position will likely expose them to blood and body fluids. Employees will be oriented to and required to understand and follow universal precautions for infection control. Qualifications:

Minimum Education:  A Bachelor’s degree in Clinical Field, Health, Business Administration or a related field is required, Master’s Degree preferred.

Minimum Experience: 

-Two years of healthcare operations leadership experience. 

-Working knowledge of Microsoft Office Suite, including Outlook, Work, Excel and PowerPoint.

Skills: 

-Experience in comfortably managing business relationship. 

-Exceptional emotional intelligence and ability to manage needs of patients across diverse functions. 

-Ability to analyze and interpret complex data. 

-Excellent problem solving, active listening skills, and ability to remain calm in stressful situations. 

-Attention to detail with strong organizational skills and ability to prioritize multiple tasks. 

- Self-starter, ability to take initiative and work independently. 

-Excellent communication skills (verbal and written). 

-Flexible. 

-Strong understanding of budget practices.
