Reporting Directly to the Virtual Hospital Director of Nursing, The Control Center Staff RN is responsible for providing high-quality patient care to various patient populations within the framework of the Medically Home model

Job Description:

Essential Duties & Responsibilities including but not limited to:

  • Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home
  • Navigating patients and families across the continuum of the Medically Home phases of care
  • Monitoring and responding to biometric data per Medically Home protocols
  • Conducting home and audio/visual patient care visits as directed by the care team and/or protocol
  • Ensuring all interventions in the patient’s care plan are implemented efficiently with a patient centered approach
  • Leveraging the resources and expertise of the Medically Home team and supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation
  • Adhering to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care
  • Promoting professional practice and a culture of safety; willingness to engage in process improvement efforts


Clinical Roles and Responsibilities:

  • Providing protocol driven assessment, care coordination and interventions as appropriate informed by Medically Home protocols for diverse patient populations with a focus on oncology, medicine and post- operative care
  • Exhibiting sound clinical decision making and critical thinking skills
  • Maintain and practice in accordance with nursing guidelines per state and hospital regulations
  • Facilitating admissions to Medically Home utilizing MH guidelines
  • Serving as a role model for professional nursing practice
  • Utilizing nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problems
  • Sharing expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertise
  • Supporting research efforts to advance knowledge and promote evidence-based practice
  • Preforming other duties and responsibilities as assigned by the Senior Director of Nursing

Quality and Safety Responsibilities:

  • Ensure compliance with reporting requirements for Sentinel Events and Serious Incidents
  • Participate in quality and safety investigations and root cause analyses
  • Take appropriate action to ensure patient safety as a result of any investigation; and
  • Serve as a resource in quality improvement processes and technique

Organizational Requirements:

Maintains strict adherence to the Lahey Clinic Confidentiality Policy. Incorporates Lahey Clinic Guiding Principles, Mission Statement and Goals into daily activities. Complies with all Lahey Clinic Policies. Complies with behavioral expectations of the department and Lahey Clinic. Maintains courteous and effective interactions with colleagues and patients. Demonstrates an understanding of the job description, performance expectations, and competency assessment. Demonstrates a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer Service standards. Participates in departmental and/or interdepartmental quality improvement activities. Participates in and successfully completes Mandatory Education. § Performs all other duties as needed or directed to meet the needs of the department.

Minimum Qualifications:

Education:

  • The minimum post-secondary education requirement to become a Registered Nurse is a Bachelor of Science degree

Licensure, Certification, Registration:

  • A RN current license

Experience

  • Three (3) years RN experience
