Job Description:

Gloucester, MA Located on Cape Ann, conveniently 50 miles northeast of Boston. Addison Gilbert Hospital is a 79-bed medical acute care facility that provides inpatient and outpatient care to residents of the Cape Ann Community.

We are currently looking for a Social Worker to join the Senior Adult Unit department at Addison Gilbert in Gloucester! The Senior Adult Unit (SAU) provides acute inpatient psych/med care for older adults. Average length of stay is 10 days. This is a voluntary, restraint free unit which is licensed by DPH. It is not considered a mental health unit licensed by DMH.

Responsibilities:

  • Assumes responsibility for the planning and delivery of safe and therapeutic treatment to all patients.
  • Collaborates with SAU multi-disciplinary treatment team to enhance the smooth delivery of quality care and development of appropriate and therapeutic treatment plans.
  • Advocates for patient needs by conducting ongoing psychosocial needs assessments and communicating these needs to the treatment team.
  • Collaborate with the treatment team, assumes responsibility for assigned patients’ disposition planning.
  • Works with other SAU Social Workers/Clinical Coordinator to provide effective communication regarding treatment and discharge planning with family, patients, HCPs or Guardians.
  • Coordinate therapeutic group programming for a diverse (socially and diagnostically) patient population of geriatric patients by assessing psycho social needs and abilities.
  • Collaborate with the treatment team, plans and provides appropriate individual and treatment groups to meet the ongoing patient needs.
  • Research and investigate appropriate group formats and purposes.
  • Provide assistance to Clinical Associates, SW and Nursing for activity ideas.
  • Makes Suggestions to unit leadership regarding purchasing of activity supplies etc.
  • Develops a weekly group/activity schedule.
  • Works collaboratively with Rehab Services to provide group/support which will enhance patient functional improvement

Qualifications:

  • Masters Degree in Social Work
  • Current Massachusetts Licensed Clinical Social Worker ( LCSW ), LICSW Preferred
  • Experience in Gerontology
  • Theoretical as well as practical knowledge of the clinical area

Schedule:

  • Monday-Friday full time days
