Under the direction of the Director, Health Care Quality and Patient Safety, the Manager, Patient Safety is responsible for the ongoing design, development, and implementation of the hospital’s patient safety program to meet the requirements of the Board of Registration in Medicine’s Patient Care Assessment Program as well as regulatory/accrediting standards including but not limited to the Joint Commission, CMS Conditions of Participation, and other regulatory agencies. Following the principles of high reliability, this individual will identify, evaluate, make recommendations and oversee implementation of risk reduction strategies and improvement efforts that mitigate actual and potential harm in the delivery of care to patients. The Manager, Patient Safety will work in collaboration with clinical and non-clinical staff to support the goals of patient safety at various departmental, organizational and system levels.

As assigned by the Director, Health Care Quality and Patient Safety, the managers works closely with medical leadership and other healthcare quality department staff to faciliate and direct peer review processes, collaborative care reviews, external reporting of safety and quality reports, communication with the hospital’s Patient Safety Organization (PSO), participation in the hospital’s Communication, Apology and Resolution Program as well as supporting the development and implementation of performance improvement opportunities.

Job Description:

DUTIES AND RESPONSIBILITIES (FTE 32- Hour Work Week)

Manager, Patient Safety Specific Responsibilities:

  • Collaborates with the involved Department leaders in the evaluation and investigation of identified risk, adverse and patient safety event notifications.
  • Participates in and acts as a resource to clinicians in the hospital’s Communication, Apology and Resolution (CARe) program.
  • Supports the Director’s activities and communications with the hospital’s liability carrier and external counsel.
  • Investigates and analyzes actual/potential risks in the institution. Develops strategies to mitigate risk for the organization.
  • Facilitates collaborative care analysis processes for the hospital, including those associated with the organization’s adverse patient event program.
  • Supports the Director and works closely with the Quality Nurse Specialists to ensure the timely preparation, development of case review summaries for medical staff and other clinical based committees and ensures receipt of feedback documents.
  • Collaborates with the Director, Healthcare Quality and Patient Safety to facilitate the reporting of Serious Quality/Sentinel/Adverse/Reportable Events to external regulatory agencies, including but not limited to BORM, DPH, and TJC etc.
  • Collaborates with the Director, Health Information Management/Privacy Officer, CIO, Compliance and Human Resources in the investigation and resolution of allegations of Breach of Patient/Employee Policy and Procedure on Confidentiality.
  • Attends and actively participates in meetings/conferences and other forums where patient safety issues are discussed.
  • Analyzes data utilizing informal or formal analytical methodology such as High Reliability, Root Cause and Failure Mode and Effects Analysis.
  • Recommends specific actions to eliminate or mitigate risk of patient harm.
  • Recommends process improvements.
  • Under the direction of the Director, facilitates performance improvement activities and teams
  • In collaboration with medical staff and hospital leadership develops and assists in the implementation of action plans.
  • Works collaboratively with the Director, Patient Experience to ensure the timely and appropriate response to patient grievances.
  • Serves as a resource to Health Care Quality, Medical, and Nursing staff on matters relative to care delivery and patient safety matters.
  • Develops plans and conducts ongoing in-service educational programs relating to patient safety activities.
  • Develops safety related policies, procedures, forms and processes.
  • Assists the Director, Health Care Quality and Patient Safety in the analysis of trends relating to data reports and findings, as well as assists in the development of corrective action plans.
  • Complies with Beth Israel Deaconess Hospital-Milton’s performance standards relating to service excellence.
