Reporting to the Director of Quality & Safety Operations, you will assist patients and families with gaining access to the variety of social services they need to improve their health and well-being. You will also engage the primary care team to increase understanding of how the social determinants of health impact health outcomes and will collaborate with the clinical team to promote health and wellness. You will train and coach CHWs using a variety of educational tools including observation, didactic training, and feedback. You will also represent the community health team at internal and external meetings related to social determinants of health screening and follow up
Job Description:
ESSENTIAL DUTIES & RESPONSIBILITIES (including but not limited to):
· Connect with patients to assess their resource needs while building a trusting relationship and communicating BILHPC’s mission and values. Provide a wide range of support to patients and families seeking community based resources, activities and benefits. Assist with access to concrete resources and follow up with patients to ensure successful connection(s) with services. Record patient care management information in the EHR (training provided) and other software no later than 24 hours after patient contact.
· Act as a patient advocate and liaison between the patient/family and community-based/government agencies and the BILHPC primary care team. Attend regular staff meetings, trainings and other meetings as requested. Collaborate with the BILHPC primary care team to ensure that patients have comprehensive and coordinated care.
· Provide culturally competent support and assist patients in connection to culturally and linguistically appropriate resources. Demonstrate up-to-date knowledge about community resources appropriate to needs of patients/families. Develop and revise resource toolkits for primary care practices.
· Provide technical assistance and training to primary care staff around administration of Social Determinants of Health Screening.
· Provide training and coaching for community health staff related to standard processes, community resources, and working effectively and compassionately with patients.
· Develop agenda for and facilitate monthly community health team meeting. Present at internal and external meetings on community health team and social determinants of health initiatives.
· Serve as co-planner and on team performance reviews. Collaborate with team members on performance improvement plans tied to caseload management.
· Monitor individual and team caseloads to ensure balanced distribution and efficiency. Track and analyze monthly case volume, identifying key themes and trendsPartner with practice managers to manage and reduce inappropriate referrals
· Develop and maintain standard work tools, including scripts and guidelines, to support team consistency and quality. Design and optimize referral workflows to improve efficiency and patient outcomes. Provide support during technology transitions and implementations
QUALIFICATIONS:
· High School diploma or GED required. Bachelor's degree preferred.
· 3-5 years related work experience required.
· Ability to initiate and maintain positive working relationships with team members, medical practice/practice staff, and other organizations.
· Understand the community served - community connectedness.
· Ability and willingness to provide emotional support, encouragement and motivation to patients.
· Advanced skills with Microsoft applications which may include Outlook, Word, Excel, PowerPoint or Access and other webbased applications. May produce complex documents, perform analysis and maintain databases
· Licensure, Certification, Registration: None
· Preferred Qualifications:
o Successful completion of a Community Health Worker formal training program preferred.
o Bilingual skills in Spanish, Portuguese, Russian or Vietnamese
OTHER REQUIREMENTS:

· Demonstrated commitment and actions to valuing diversity and contributing to an inclusive working and learning environment
