In addition to the duties of a staff physical therapist, the Clinical Team Leader is primarily responsible for improving clinical excellence of the staff. The three major areas of responsibility are: orientation and on-going coaching of staff to improve clinical skills, ensuring that staff meet department documentation requirements through coordination of the peer chart review process and improving clinical outcomes through individual coaching and active participation in the department’s performance improvement initiatives. The individual in this position provides feedback regarding staff performance in the areas of responsibility to the site manager. The Clinical Team Leader also plays an active role in ensuring ongoing annual competency of the clinical staff. This individual provides oversight of daily operations in the absence of the site manager.
It is recognized that the time commitment to perform these additional duties will vary depending on staffing trends but should average 2-4 hours per week.

Job Description:

Education/Experience

College graduate with a minimum of a BS in a rehabilitation field (MS preferred) with 3-5 years of clinical experience.  Prior supervisory or teaching experience preferred.

Certification/License:  Current Massachusetts license in respective field.  BCLS

Essential Responsibilities:

Patient Care: Provides quality care for patients according to national, state, hospital and department standards of care

  • Performs a complete/thorough evaluation of patient’s chief complaint, physical impairments, activities of daily living and functional limitations.
  • Develops a comprehensive plan of care which includes appropriate, measurable and functional goals
  • Plans and implements appropriate treatment activities, effective in improving identified physical impairments and functional limitations.
  • Provides on-going reassessment and modification of the treatment plan in response to patient’s progress.
  • Provides appropriate discharge planning and patient/family teaching including provision of home programs, recommendations for self-care and adaptation to disability if needed.
  • Provides therapy services in a cost-effective manner.
  • Uses department resources appropriately to ensure the highest quality of care.
  • Completes evaluation/flow sheet documentation immediately following patient treatment which includes objective findings, treatment specifics, response to treatment and patient education in accordance with department standards
  • Completes periodic patient progress notes that documents patient’s current status, progress towards established goals and treatment plan modifications that support the need for the continuation of skilled services in accordance with department/state regulations.
  • Completes discharge progress note within established timeframe that includes patient’s status, progress towards established goals and discharge disposition.
  • Demonstrate effective overall practice management strategies to ensure that patients are not lost to follow-up, scheduled visits are commensurate with need for skilled services and patient compliance with attendance policy.

Financial Management

  • Completes/submits charge sheet at the end of each work shift that accurately reflects services rendered for each patient in accordance with dept. standards/Medicare regulations
  • Maintains current knowledge of insurance regulations/requirements and limitations of reimbursement for professional services
  • Obtains insurance authorizations for all services rendered. Maintains accurate tracking of visits inclusive of number of visits and timeframe of authorization.
  • Completes/submits all necessary paperwork to third party payers for initial approval, continuation of services and appeal of denied services.

Supervisory Responsibilities

  • Participates in the orientation and initial competency training of new hires as delegated by site manager.
  • Responsible for consistently coach clinical staff towards excellence in patient care: including clinical skills, documentation and outcomes.
  • Provides input regarding on-going clinical competency/skills of staff and recommends commendation or disciplinary action plan as indicated
  • Assists in the verification of on-going competencies of staff members as requested by department leadership.
  • Provides feedback to staff regarding documentation practices and clinical reasoning skills based on peer chart reviews.
  • Responsible for oversight of peer chart review process including involving all professional staff in monthly chart review process, tracking and interpreting data for practice/individual trends and completion of quarterly report for site manager/department director. Identifies trends that fall below baseline and mentors corrective actions.

Department Development Responsibilities

  • Identifies areas for clinical and/or operational improvement for department and offers potential solutions.
  • Collects and reports on performance improvement initiatives as requested by rehab management team.
  • Actively encourages the clinical staff to integrate new skills/training into overall patient management.
