Manage and monitor HIV primary care patients including education, screening, immunizations. Assist/support MD during patient visits for continuity or care. Assist with writing/editing grants for purposes of funding the ACCESS program. Responsible for patient level data management and HRSA reporting requirements.

Job Description:

Essential Duties and Responsibilities:

Clinical

  • Manage and monitor HIV primary care patients including education, screening, immunizations
  • Assist/support MD during patient visits for continuity or care
  • Triage clinical phone calls, medication requests, scheduling needs
  • Provide patient education about HIV disease and STI prevention, HIV disease progression, HIV treatment options, chronic disease management and lab results
  • Administer immunizations, injectable medications or treatment medications as ordered by MD
  • Responsible for prior authorizations for medications/procedures as needed
  • Collaborate with and assist MD in providing HIV post exposure consultation and care
  • Responsible for reporting HIV/STI cases to Mass DPH
  • Document client interactions in Epic (electronic medical record system)

Administrative

  • Assist with writing/editing grants for purposes of funding the ACCESS program
  • Responsible for patient level data management and HRSA reporting requirements
  • Provide training and direct supervision to the Medical Case Manager and Counselor/Tester
  • Coordinate and implement weekly HIV team meetings with staff
  • Develop, maintain and report on Continuous Quality Management Projects
  • Participate in hospital Continuing Education programs and coordinate at least 1 HIV related program for Friday morning lecture series

Experience/Knowledge/Skills Required

  • Minimum of Bachelor’s degree in nursing
  • Familiarity with Epic electronic medical record system preferred
  • Experience providing services to people of diverse social-economic backgrounds, ethnicities, sexual orientation and lifestyles
  • Ability to organize, prioritize multiple projects and meet deadlines in a time sensitive environment
  • Computer proficiency in Microsoft Office
  • Ability to communicate effectively and professionally
  • MA Driver’s License required
