Job Description:

Duties/Responsibilities:

  • Coordinates care for patients preparing to start treatment including but not limited to: pre-treatment labs, imaging, referrals, venous access placement, treatment plans.
  • Collaborates and communicates professionally with multidisciplinary team including oncologists, radiation oncology, nutrition, social work, spiritual care etc, to provide comprehensive care to our medical oncology patients.
  • Act as a liaison between patients, families, caregivers, and providers to optimize patient outcomes.
  • Act as point of contact between inpatient and outpatient oncology patients to facilitate timely discharge, appropriate outpatient follow-up, pre-treatment education, and act as liaison for oncologists and hospitalists/in-house providers.
  • Establishes and coordinates a supportive care plan, and development of survivorship care plans as recommended by the Commission on Cancer.
  • Active participation in palliative care, and multidisciplinary team meetings.  Function as the outpatient clinical representative to identify and refer any patient(s) who could benefit from palliative and/or multidisciplinary intervention(s)
  • Provides patient/family education regarding chemotherapy/immunotherapy utilizing current literature as updated/edited through your professional review of approved professional sources.
  •  Work closely with concurrent patients and multidisciplinary team to help streamline the treatment process, appointments, and address obstacles.
  • Assesses the physical and psychosocial status of clients by means of interview, health history, physical examination, and diagnostic studies.
  • Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow-up.
  • Completes objective and accurate documentation of patient care in the electronic health record
  • Establishes relationships with professional and/or other health-related groups within the community, and shares clinical expertise with professional and supportive personnel.
  • Comprehends and adheres to the policies and standard operating procedures of the organization.  This includes, but is not limited to attendance guidelines regarding timeliness of notification, and promptness in reporting for duty.
  • Collaborates with the oncology leadership team to compose, update, and/or edit the department-based policies and procedures
  • Collaborate in organizing community oncology events with oncology team and leadership to support our commitment of bringing cancer care awareness and programs to those most in need.
  • Demonstrates professionalism, accountability, and positive leadership qualities
  • Assumes professional responsibility for current practice and professional growth.

Education/Experience Required:

  • Evidence of continuing education.
  • Demonstrated proficiency in basic nursing care.
  • Demonstrates an ability to assess patient’s needs, make appropriate referrals.
  • Ability to communicate and work effectively with the physical and emotional developments of all age groups.
  • Experience in program development.
  • Knowledge of community resources
  • Demonstrates ability to work in a team setting
  • Interact effectively with members of the interdisciplinary team, including physicians, as well as external customers, to achieve desired clinical service and financial outcomes.
  • Leadership skills including group facilitation and conflict management
  • Ability to set priorities and meet deadlines
  • Multi-tasking skill is critical
  • Neat and well-groomed appearance
  • Broad knowledge of community health concepts, services and resources.
  • Capable of comprehending, executing, acting, and directing independently under the established guidelines of the position.
  • Interpersonal skills

Qualifications:

  • Two years of recent oncology nursing experience
  • Graduate - approved School of Nursing
  • HS Diploma
  • BSN Preferred, MSN Preferred

Registration/Certification:

  • Current RN License in Massachusetts.
  • Oncology Certification in Nursing (OCN) preferred
  • Chemo/Bio Administration Certification

Physical/Environmental Requirements

This job requires frequent bending, carrying, pulling, pushing, reaching, standing, stooping, walking and lifting over 10 lbs. There will be occasional need for climbing stairs, lifting in excess of 50 – 100 lbs and sitting. Constant use of hearing, sight and speech. Frequent use of smell and touch. Frequent keyboard and telephone use. Constant concentration and problem solving.
