The Home Infusion Pharmacist acts as both a clinical & operational representative of the Home Infusion Pharmacy to ensure that the products and services offered to patients meet or exceed the standards outlined by state Boards of Pharmacies, Federal Regulations, as well as the United States Pharmacopeia. The Home Infusion Pharmacist will work as part of an interdisciplinary team to ensure the delivery of appropriate, safe & timely compounded medications, non-compounded medications & infusion supplies.Job Description:
Primary Responsibilities:
1. Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug Administration, the Drug Enforcement Administration, and the United States Pharmacopeia.
2. Maintains certification and credentials in accordance with the Board of Registration of Pharmacy and adheres to policies and procedures.
3. Reviews and verifies medication orders.
4. Provides education on medication & their administration to patients, their caregivers as well as nursing staff.
5. Creates patient care plan & documents progress towards goals.
6. Collaborates as part of an interdisciplinary team to advocate for patient outcomes & adherence to therapies.
7. Reviews / interprets lab orders to make decisions on medication changes or recommendations on medication changes.
8. Documents and communicates pertinent patient information and discussions with the healthcare team and pharmacy staff.
9. Compounds, checks and verifies sterile compounded products. Prepares, checks and verifies non-sterile products, pumps and supplies.
10. Supervises the activities of students, technicians, and residents within the pharmacist area of practice.
11. Participates in the onboarding and training of new hires and precepts pharmacy students and/or residents through core clinical rotations or learning experiences.
12. Participates in department initiatives including: Performance Improvement, Policy & Procedure review, Workflow Assessment & Enhancement, and Financial Performance.
13. Participates in on-call rotation.
Required Qualifications:
• Active MA Pharmacist Registration.
• 3 Years Sterile Compounding Experience.
• Ability to develop strong relationships with diverse groups of professionals.
• Experience with computer systems required, including web-based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.
Preferred Qualifications:
• Completion of PGY-1- Pharmacy Residency
• Board Certification- Compounded Sterile Preparations

• Problem Solving: Ability to address problems that are varied, requiring analysis or interpretation of the situation using direct observation, knowledge and skills based on general precedents.
• Independence of Action: Ability to follow precedents and procedures. May set priorities and organize work within general guidelines. Seeks assistance when confronted with difficult and/or unpredictable situations. Work progress is monitored by supervisor/manager.
• Written Communications: Ability to communicate clearly and effectively in written English with internal and external customers.
• Oral Communications: Ability to comprehend and converse in English to communicate effectively with staff, patients, families and external customers.
• Knowledge: Ability to demonstrate full working knowledge of standard concepts, practices, procedures and policies with the ability to use them in varied situations.
• Team Work: Ability to work collaboratively in small teams to improve the operations of immediate work group by offering ideas, identifying issues, and respecting team members.
• Customer Service: Ability to provide a high level of customer service to patients, visitors, staff and external customers in a professional, service-oriented, respectful manner using skills in active listening and problem solving. Ability to remain calm in stressful situations.
• Adaptability: Ability to work in a fast-paced environment and adapt to acute change.
Social/Environmental Requirements:
• Work requires close attention to task for work to be accurately completed. Intermittent breaks during the work day do not compromise the work.
• Work routine is fairly consistent, but employee needs to be able to use judgment to respond to events several times a week.
• Potential exposure to adverse environmental conditions
Daily: Exposure to dust, gas, fumes, steam, chemicals, Protective equipment required (Respirator, earplugs, mask, gloves, eyewear etc.).
• Health Care Status: HCW 3: Regular provision of service in a patient care area. Examples: Unit coordinators, lead coordinator/supervisor, patient liaison, nutrition services, environmental services, registrars, information desks, security, etc- Health Care Worker Status may vary by department
Sensory Requirements:
Close work (paperwork, visual examination), Color vision/perception, Visual monotony, Visual clarity feet, Conversation, Monitoring Equipment, Telephone, and Background Noise.
Physical Requirements:
Medium work: Exerting up to 50 pounds of force occasionally and or up to 20 pounds of force frequently. Job is physical in nature and employee needs to stand and/or move around through the majority of their shift.
This job requires constant walking, Endurance-working up to 4-6 hours without a break, frequent sitting, standing, twisting neck, twisting waist, Keyboard use, Reaching-above shoulder height, Reaching-below shoulder height, Lifting and carrying items weighing up to 10 lbs, pushing items weighing up to 10 lbs. There may be occasional bending neck, bending waist, Fine Manipulation using both hands, Pushing/Pulling using both hands.
