Job Description:

What You’ll Do

Responsible for the oversight of a clinical interdisciplinary team and leading the team in the provision of high quality, effective, and efficient clinical services. Promote quality in patient care delivery in accordance with agency objectives, policies, and procedures. Supervise, support, and educate staff at the point of service in the field.

More Specifically:

  • Oversee an interdisciplinary team and the assignment of care, clinical integrity of practice, and flow of documentation through supporting team members.
  • Encourage and foster a positive environment among the team toward establishing and maintaining a culture of patient-focused service, including employee feedback and staff engagement.
  • Provide oversight and mentoring in the field related to the accurate capture of the clinical and functional impairment of each patient as documented in the OASIS-C document.
  • Provides direct supervision of an Interdisciplinary team of RNs, LPNs, PTs, PTAs, OTs, STs, MSWs, HHAs, and Clinical Scheduling Specialist.
  • Accountable for overall team performance in the areas of productivity, quality outcomes, patient & family satisfaction, OASIS integrity performance, compliance & risk management.

What You’ll Need:

  • Registered Nurse from a Graduate of National League for Nursing accredited school of nursing
  • BSN strongly preferred;
  • Current valid MA nursing license;
  • Current American Heart Association (AHA) CPR certification;
  • 1-2  years’ experience managing a team of clinicians in a home health setting;
  • Knowledge of Medicare/Medicaid regulations in home health;
  • Strong interpersonal skills and the ability to facilitate cooperation and communication among team members.
